New Patients

What to Expect from your First Visit

Prior to your first visit with us you will need to fill out new patient forms and provide your insurance information. These forms may be downloaded here. You will be asked to check in to the front desk where your identification and insurance cards will be scanned into our system and we will take co-payment, if applicable. Our medical staff will ask you for medical information which may include your current medications and allergies, height and weight, and a urine sample. Your first visit may take between 10 and 60 minutes with your physician, not including check in. Future visits, lab tests and diagnostic studies may need to be scheduled following your appointment.

Appointments

We always do what we can to keep appointments on schedule. However, due to emergencies and varying patient needs, sometimes delays are unavoidable. We ask for your patience and understanding in these situations.

If you must cancel or change an appointment, please notify us immediately. There is never any charge or fee for a cancelled appointment. Our Scheduling Department is available to take your calls 8:00am – 5:00pm Monday through Friday. To reach our schedulers, call us at 503-561-7100, and press option 1.

Prior to your first appointment, you will need to obtain a referral from your primary care physician. Following our receipt of the referral, we will schedule your first appointment and, if time allows, mail you paperwork that must be filled out prior to your seeing the doctor.

Please arrive early for all appointments. Bring your current insurance card, a list of all medications you are currently taking, (including dosage), as well as information on relevant address, telephone number, and a photo ID. Please also keep in mind that the doctor may require a urine sample during your visit.
