What to Expect from your First Visit:

Prior to your first visit with us you will need to fill out new patient forms and provide your insurance information. These forms may be downloaded here. You will be asked to check in to the front desk where your identification and insurance cards will be scanned into our system and we will take co-payment, if applicable. Our medical staff will ask you for medical information which may include your current medications and allergies, height and weight, and a urine sample. Your first visit may take between 10 and 60 minutes with your physician, not including check in. Future visits, lab tests and diagnostic studies may need to be scheduled following your appointment.

Appointments

We always do what we can to keep appointments on schedule. However, due to emergencies and varying patient needs, sometimes delays are unavoidable. We ask for your patience and understanding in these situations.

If you must cancel or change an appointment, please notify us immediately. There is never any charge or fee for a cancelled appointment. Our Scheduling Department is available to take your calls 8:00am – 5pm, Monday through Friday. You can reach the schedulers at 503-561-7100 option 1.

Prior to your first appointment, you will need to obtain a referral from your primary care physician. Following our receipt of the referral, we will schedule your first appointment and, if time allows, mail you paperwork that must be filled out prior to your seeing the doctor.

Please arrive early for all appointments. Bring your current insurance card, a list of all medications you are currently taking, (including dosage), as well as information on relevant address, telephone number, and a photo ID. Please also keep in mind that the doctor may require a urine sample during your visit.

Prescription Information

To refill your prescription, please contact your pharmacy. Your pharmacy will contact our physicians about your request. Please allow at least 2 business days for your refill to be completed. If you need your prescription for the weekend, please be sure to have your request sent to us no later than Thursday evening. If you change your pharmacy, be sure and let us know to avoid delays in processing your prescription.

Insurance/Payment

Our office does require payment, if applicable, on the day of your visit. You may choose to pay for the visit in full upon arrival, but you are required to pay your insurance’s copay on the day of the service. If you are uninsured, we have financial assistance available. Please ask for an application. All fees for outside labs, pathologies, or imaging must be discussed and paid with their respective billing departments.

Insurance

Policy coverage often varies from one plan to the next, as do fees. Our fees are accepted by most plans but sometimes the fee exceeds “usual customary and reasonable fees” established by your insurance company. Should a dispute arise over these fees, please call your insurance carrier. We accept many commercial plans, Medicare, Marion Polk OHP, DMAP, and are contracted with many insurance companies. Medicare

We are a participating provider in the Medicare program. As a Medicare patient, you must still pay your yearly deductible as well as the 20% of Medicare’s approved amount.

Financial Assistance

If you are uninsured or have difficulties paying your coinsurance or deductible, we may be able to help. Our financial assistance program may provide assistance for up to 100% of your bill. To apply for the financial assistance program, you will need to provide us with bank statements, federal tax returns, and any other relevant income information as well as fill out the application. Elective Procedures

Procedures not considered medically necessary by your insurance company, such as vasectomy & circumcision, must be paid in full at the time of the procedure. If possible, we will contact your insurance to verify coverage and provide you with an estimate of your financial responsibility. We will request an authorization for your procedure if one is needed
